Osteoporosis and osteoarthrosis: pathogenetically interrelated diseases? (Review of literature)

Abstract

Data about a relationship between osteoporosis (by the index of bone mineral density, BMD) and osteoarthrosis are presented on the basis of an analysis of scientific literature. The majority of the analysed works revealed an inverse relation between these diseases, i.e., patients with gonarthrosis and coxarthrosis demonstrated high BMD values in the regions of their lumbar spine and proximal femur. Patients with spondyloarthrosis and osteoarthrosis of small joints of hand occasionally developed a relation of the above pathology with higher BMD. Many authors point out that, despite higher BMD values in osteoarthrosis, the risk of fractures significantly exceeds the population risk. There is no doubt that the above fact necessitates taking of measures for prevention of osteoporotic fractures in patients with the “normal” and even “increased” BMD, rather than with the reduced one only. Assessing the “subchondral bone – osteosrthrosis” relationship, the authors have analysed four clinical situations, which combine these diseases. The problem of investigation of common pathogenetic mechanisms in the development of osteoarthrosis and BMD requires further studies with regard for biological, molecular and biomechanical factors, which produce their effect on the relationship between the subchondral bone and articular cartilage. The disclosure of the mechanisms of relationship between osteoarthrosis and osteoporosis, the revealing of high risk groups, the use of modern diagnostic methods and purposeful prophylaxis of fractures will contribute to improving the quality of life in patients of older age groups.
